INFORMED THE PERSON-IN-CHARGE OF THE COMPLAINT AND INVESTIGATED THE ISSUES PRESENTED BY THE COMPLAINANT.
NOTES: (1) THIS FACILITY HAS TWO RESTROOMS. ONE RESTROOM IS NOT BEING USED AS A RESTROOM, BUT IT IS FUNCTIONING AS A STORAGE UNIT. ALL EMPLOYEES KNOW NOT TO USE THE STORAGE ROOM-RESTROOM. IN ADDITION, THE OTHER RESTROOM IS A REGULAR RESTROOM AND IT IS CLEAN AND WELL MAINTAINED. (2) NO FRUIT FLIES WERE OBSERVED DURING THE INVESTIGATION. HOWEVER, THE PIC HAS STATED THAT STICKY-TRAP STRIPS WILL BE PLACED NEAR THE BACK DOOR. THIS DOOR IS CONSTANTLY BEING OPENED AS DELIVERY DRIVERS AS THEY PICK-UP FOOD ORDERS. (3) THIS ESTABLISHMENT DOES HAVE A NO SMOKING/VAPING POLICY. WRITTEN FORMS WERE PROVIDED TO ALL EMPLOYEES TO ENSURE COMPLIANCE. DURING THE INVESTIGATION, SMOKING-RELATED VIOLATIONS WERE NOT OBSERVED. THE OWNER PLANS TO RE-EDUCATED STAFF OF THE SMOKING POLICY. FOLLOW-UP WILL OCCUR ON JULY 25, 2022 FOR THE PEST TRAP PLACEMENT. |
